Wuhan is a central Chinese city built where the Han meets the Yangtze, and it has one of the largest student populations of any city in the world. That density is the point: a very large share of the international, and specifically South Asian, medical cohort in China is here or in cities like it.
Wuhan University and Huazhong University of Science and Technology are both major national institutions, and both run substantial medical and engineering programmes with English-taught provision for international students.
Costs are markedly lower than the tier-1 cities on both tuition and living. The honest trade is that Wuhan has no municipal graduate work scheme of the kind Shanghai and Beijing operate, and its summers are famously, genuinely punishing.

Climate

A growing metro network across three historic urban centres separated by rivers. Journeys are longer than the map suggests.
Housing
University dormitories are the standard option for international students and are inexpensive. Ask for the actual rate rather than an estimate.
Working
The same national restriction as everywhere in China — approval plus an endorsement, and not something to budget on.
Language
Substantially less English than Shanghai. Clinical rotations run in Chinese with Chinese-speaking patients, which is the reason HSK matters for medical students specifically.
The cohort
A large and long-established South Asian student community, which makes arrival easier and makes it easier to never learn Chinese. Both are true.
Whatever you save on tuition, this does not move: a Chinese MBBS only leads to practice in Nepal if the university is on the Nepal Medical Council's recognised list and you pass its licensing examination
And what a tier-2 city does not have: some cities including Shanghai, Beijing and parts of Guangdong have introduced easier work routes and internship allowances for international graduates of Chinese universities
This is the trade stated honestly. Wuhan is meaningfully cheaper and hosts genuinely strong universities, and for a student who intends to return to Nepal and license through the NMC, the absence of a municipal work scheme costs nothing at all. What would cost you everything is choosing a cheaper institution that the Nepal Medical Council does not recognise — so make the recognition check first and let cost decide between the options that survive it.
